Deep Space Mind 215 is a community-based mental health co-operative. This page explains what happens to information you provide through this website, and which other companies are involved when you use it.
What this website collects
This website itself does not collect or store personal information. It has no accounts, no login for visitors, and no comment forms. There are three places where you can choose to send us information, and each one is handled by a different company.
The service request form
The service request form on our Contact page is provided by Jotform. When you fill it in, your answers go directly to Jotform and are stored there. They do not pass through or get stored on this website.
Jotform holds a Business Associate Agreement with Deep Space Mind 215, which is the arrangement required when a company handles health-related information on our behalf.
Jotform sets two cookies in your browser as soon as the Contact page loads, before you type anything or submit anything. They are named userReferer and guest, and they last about one month. They belong to Jotform, not to this website.
The Synapses newsletter
Our newsletter is published on Substack. If you enter your email address in a newsletter signup box, that address goes to Substack and is governed by Substack’s own privacy policy. Signing up for the newsletter is a separate choice from making a service request — neither one subscribes you to the other. You can unsubscribe from any Synapses email at any time.
Donations
Donations are processed by ActBlue on their own website. No payment information is entered on, or passes through, this website.
Website measurement
This website is hosted on WordPress.com, and uses its built-in visitor statistics to count page views. That measurement does not set cookies and does not identify you individually.
What this website does not do
Verified against the live site on 10 September 2026. This website carries:
- No advertising networks
- No behavioural tracking pixels
- No data brokers
- No third-party analytics beyond the hosting platform’s own cookieless page-view count
Apart from the hosting platform, the only outside companies your browser contacts while using this website are Jotform, on the Contact page, and Substack, if you follow a link to our newsletter.
Pending legal review
The following require confirmation by Deep Space Mind 215’s counsel and are not yet stated on this page:
- Whether and how HIPAA applies to information collected here, and the full Notice of Privacy Practices that follows from it
- Whether 42 CFR Part 2 applies to any service category, and the separate consent that would require
- Obligations under the Pennsylvania Mental Health Procedures Act
- How long information is kept, and how it is disposed of
- Your rights to see, correct or delete information you have given us, and how to exercise them
- The named Privacy Officer responsible for responding to a privacy concern or a breach
